  • Application with prospectus can be obtained by paying Rs. 130/- by cash or by sending D.D/MO in favor of The General Secretary, Ezhuthachan National Academy ,payable at Trivandrum .
  • Applications down loaded from the website are to be submitted with a DD for Rs 100/- drawn in favour of the General secretary, Ezhuthachan National Academy , payable at Trivandrum
  • Duly filled up application has to be submitted with all relevant documents to the The General Secretary, Ezhuthachan National Academy ,payable at Trivandrum

  DOCUMENTS TO ACCOMPANY THE APPLICATION

Certificate in proof of Age.

Certificate in Proof of Income( if eligible).

3

Mark List of the Qualifying examination.

4

Transfer Certificate and Pass Certificate( at the time of dmission).
5

Certification respect of Reservation under SC/St/OBC.
6

Medical fitness Certificate from a medical officer not below the rank of a Civil Surgeon in the Govt of Kerala.
7

Certificate in proof of any other claim made in the application form.

  • Fill in the name of the candidate in capital letters , as given in the certificate in proof of Age.
  • Fill the data carefully so that no column is left vacant.
  • The marks secured by the Applicant has to be entered as given in the Mark list copy submitted.
  • The details of fee remitted towards Application shall be entered in the Applications down loaded from the website.
  • Those seeking admission through lateral entry shall specifically state that in the application.(Such candidate shall essentially enter the details of Mark obtained in the D.Pharm Examination).
